SWEEPING reforms designed to improve the NHS could fail because of poor local management, a committee of MPs warned today.

Last year health czar Lord Darzi called for the opening of polyclinics and demanded more choice for patients, including creating personal budgets for those with long-term conditions so they could decide how NHS money was spent on them.
