EARLIER this year I developed a pain at the very end of my left jawbone, near my neck. I had just had a crown made at a Harley Street dentist but when I went back he found no evident complications. The pain continued to worsen, particularly at night. I would get into bed and the dull ache would burgeon into full- blown agony.

I rang my dentist again in desperation. "I'm sorry, I'm just off skiing," he sang happily. "It can't be the tooth, though." Clang! I felt a prison door close.
